If you apply, we collect your name, role, practice, specialty, suburb, email, phone and, if you choose, your Leak Check figures. We use them only to assess your application and contact you about the report. Applications are delivered to us by email through our form provider, Formspree.
We never ask for or accept patient information. Reports are built from counts of referrals by referrer and month, under a confidentiality agreement signed before any data is shared.
